Dr. Hagerott is accepting new patients. To schedule an appointment, please go to the Forms page and complete the History Form and the Insurance Form and submit it to our office. Once received, we will send a link to register for Simple Practice, then we will contact you to schedule an appointment.
How do I get started? A referral from your primary care physician or specialist physician is required for some insurance carriers to determine medical necessity and determine benefits. This also provides us with medical records needed to begin the evaluation. Most area doctors have our referral form, but they may also send us their own internal form, or you can send clinical notes which discuss the need for neuropsychology.
Referral Process: Once a referral is received from your physician, we will call you. Before we can schedule an appointment, you will need to complete our new patient paperwork.(History Form for New Families). Please complete the forms in their entirety, as all information is required. This will allow Dr. Hagerott to focus on your presenting concerns in the appointment, rather than filling in missing history. You may e-mail, fax, or mail the forms. It is very important that we receive prior psychological testing reports and key medical and educational records as well. (Checklist of Records Needed for the Evaluation). Once we receive your paperwork, we will send a link to register for Simple Practice, then we will contact you to schedule an appointment. Our new patient appointment times are Monday through Thursday at 8:00 a.m. Please allow two hours for this appointment. We will contact you the week before to confirm the appointment. If not confirmed, the appointment will be cancelled.

What can I do while I am waiting to see the doctor? Please gather important records which will assist in the process and upload them to Simple Practice. We suggest that you obtain information from the child’s teacher, [Found here] and a checklist of records [Found here] which will be helpful for Dr. Hagerott to review. It is necessary for Dr. Hagerott to have a copy of all prior psychological evaluations at the time of the first appointment (all private, school, or neuropsychological testing completed) and a copy of the current IEP/504 plan. We are unable to make copies of records at the time of the initial appointment. Please upload documents to Simple Practice as single documents. Don’t upload documents page by page.
How much time should be allowed for the first appointment and what should my child expect? Please allow two hours for the first appointment. Please arrive at least 15 minutes prior to all appointments as there are forms that need to be completed prior to seeing the doctor. If it is necessary to speak with the doctor without the child present, please bring another adult with you if the child is too young or would be fearful of playing alone in another room adjacent to the parent. Otherwise, the child and parent will be interviewed together. Young children will have toys available to play with. You may tell your child that this will be a play visit and the parent will talk with the doctor. Older children may want to be reassured that there will be no shots or may want to know that the goal of the evaluation is to make things easier or less stressful for them.
Insurance information: Dr. Hagerott files insurance claims for Smart Health, if it is the primary insurance. If you have Blue Cross Blue Shield or any other private insurance coverage; you may be able to self file for reimbursement from your insurance company. Contact your insurance company to determine if you have Out of Network Benefits. This means that your insurance will reimburse for services out of their contracted network.
Ask your insurance company what form you will need to complete to submit a claim and to what address the claim should be sent (forms are often available on the insurance company’s website).
You may want to ask if any pre-certification forms are required. Dr. Hagerott will complete a form for precertification in the first appointment if needed, but if presented outside of the initial appointment, there will be a charge for completion of any forms.
When you are seen in Dr. Hagerott’s office, you will be responsible for payment at the time of service. We will provide a charge ticket with service codes and diagnoses to submit to your insurance company.
When you submit your claim, be sure to keep copies of your paperwork and the date you submitted them in the event your insurance company loses the information.
We suggest that you write on the claim that the reimbursement check be sent to you, the subscriber as any payments received by our office may be returned to the insurance company.
